If your procedure has already been scheduled, or you are having a baby, you can speed up your registration process by pre-registering online. To begin the registration process, please click on the link below. This form needs to be completed at least two business days or 48 hours prior to admission or scheduled procedure.

Hospital-Based Physicians

Midland Memorial wants you to be well informed about your healthcare coverage. When you access services at a network hospital or facility, you may receive care or services from hospital-based physicians in addition to your primary care or treating physician who will bill you separately from the hospital. Anesthesiologists, radiologists, pathologists and emergency room physicians are examples of hospital based physicians.

In some cases, a hospital-based physician may decide not to participate or contract with the same insurance companies as Midland Memorial Hospital. The following link contains a list of hospital based physicians who have been granted medical staff privileges at Midland Memorial Hospital along with their office phone numbers. As a patient, you may wish to contact applicable provider offices directly to ask questions regarding your insurance and any amounts you may owe as a part of your deductible or co-insurance.

Medical Records

The purpose of Release of Information is to provide copies of patient records to various requesters (i.e. patients, attorneys, insurance companies, etc.) for varied reasons.

When writing to obtain copies of records, please provide us with the following information:

1. The patient’s name at treatment
2. Patient’s date of birth
3. Patient’s Social Security number
4. The date(s) of treatment
5. Exact information from the date(s) of treatment
6. Where the information should be sent
7. Purpose of request
8. Sign and date the request
9. Contact number
10. Copy of photo ID

Please do not send requests for information or confidential information via e-mail. We cannot honor e-mail requests because a signature is required to release medical information.

The Release of Information will not provide records without a written request by the patient, the parent of a minor patient, or a legal representative. After the request is received, Release of Information should provide an answer in 15 working days. Copies of records are faxed only for emergency medical treatment (i.e. patient is in the Emergency Room or in Labor & Delivery).

There are fees to obtain copies of records. These fees must be paid prior to records being mailed to the requester. When a request is received, Release of Information will send the requester an invoice stating the fee for copies. The fees for copying records are set by the State of Texas and are reviewed for changes annually.

Birth Certificates

The Birth Certificate Office is responsible for the completion of birth certificates of babies born at Midland Memorial Hospital. To obtain copies of birth certificates, parents should visit or call:

What Are Advance Directives?

A living will, health care proxy and durable power of attorney—are the legal documents that allow you to give direction to medical personnel, family, and friends concerning your future care when you cannot speak for yourself. You do not need a lawyer in order to complete Advance Directives.

Patient Services Advance Directives

You have the right to make decisions about your own medical treatment. These decisions become more difficult if, due to illness or a change in mental condition, you are unable to tell your doctor and loved ones what kind of health care treatments you want. That is why it is important for you to make your wishes known in advance.

Here is a brief description of each kind of Directive:

Living Will
A set of instructions documenting your wishes about life-sustaining medical care. It is used if you become terminally ill, incapacitated, or unable to communicate or make decisions. A living will protects your rights to accept or refuse medical care and removes the burden for making decisions from your family, friends, and medical professionals.

Health Care Proxy
A person (agent) you appoint to make your medical decisions if you are unable to do so. Choose someone you know well and trust to represent your preferences. Be sure to discuss this with the person before naming them as your agent. Remember that an agent may have to use their judgment in the event of a medical decision for which your wishes aren’t known.

Durable Power of Attorney
For health care: A legal document that names your health care proxy. Once written, it should be signed, dated, witnessed, notarized, copied, and put into your medical record.
For finances: You may also want to appoint someone to manage your financial affairs when you cannot. A durable power of attorney for finances is a separate legal document from the durable power of attorney for health care. You may choose the same person for both, or choose different people to represent you.

Patient Advocate

The health and well-being of our patients is our primary concern, and we will strive to make your stay with us as pleasant as possible. We encourage you to request information, offer suggestions and to express concerns and satisfaction regarding any hospital staff or service.

An issue can be resolved most promptly by speaking with your nurse or another health care professional directly involved with your care. However, if you feel an issue is not being addressed appropriately, or if you need additional assistance, please call the Patient Hotline at (432) 522-CARE or dial extension 2273, and our Patient Advocate will assist you.

For Visitors

Visitation Guidelines

Visitation Policy &Guidelines

The intent is to create a safe, private environment for all patients, visitors, and employees while allowing ample opportunity to visit friends and relatives.

  • Children under 12 must be with an adult at all times.
  • Two (2) visitors at a time in the patient’s room.
  • Wash your hands as you enter and before leaving patient’s room.
  • Due to fire and safety codes, please avoid standing and visiting in the hallways.
  • In any waiting room, please limit four (4) people for every patient.
  • Latex balloons are not allowed in the hospital.
  • Please respect all requests made by physicians and hospital staff.

Midland Memorial Hospital has the right to change these guidelines at any time. Behavior that interrupts or interferes with patient care is cause to be removed from hospital grounds. The safety of our patients, visitors, and staff is important to the family at MMH.
